Multiple RCTs show red/near-infrared light improves muscle recovery, wound healing, and pain in specific contexts (e.g., delayed-onset muscle soreness, oral mucositis, tendinopathy). Meta-analyses support efficacy for musculoskeletal outcomes. The mechanism is established in cell and animal models. Human trials are smaller and outcome-specific rather than broad, but the weight of evidence across independent groups is consistent.
Red (600–700 nm) and near-infrared (700–1000 nm) light penetrates tissue and is absorbed by cytochrome c oxidase in mitochondrial complex IV, increasing ATP (adenosine triphosphate) production and reducing reactive oxygen species. This is proposed to enhance cellular energy, reduce inflammation, and improve tissue repair.

Most RCTs are small (N < 50) and heterogeneous in wavelength, power density, and treatment duration, making dose optimization unclear. Evidence is strongest for acute musculoskeletal injury and wound healing; claims about systemic aging, cognition, or longevity lack human trial support. Publication bias toward positive results is likely. Skin tone and tissue depth affect light penetration, but few trials stratify by these factors.
