Multiple large prospective cohort studies (GRAIL's CCGA and Circulating Cell-free Genome Atlas, and independent validation cohorts) show these tests can detect 50–90% of advanced cancers with low false-positive rates (~0.5–1%), supporting clinical utility in symptomatic or high-risk populations. However, sensitivity in early-stage disease (stage I–II) remains modest (20–40%), and no randomized trial has yet demonstrated that screening asymptomatic populations with these tests reduces cancer mortality or improves outcomes compared to standard screening. Real-world performance may differ from controlled studies.
Blood-based multi-cancer detection tests (also called liquid biopsies) work by identifying circulating tumor DNA (ctDNA), proteins, or other biomarkers shed by cancer cells into the bloodstream.
