Direct oral NAD+ has poor bioavailability and almost no human outcome trials. Most human data is on NMN and NR, not NAD+ itself. RCTs are small (N=20–100), short-term (weeks to months), and often industry-sponsored. No long-term mortality or aging-rate data exists in humans. Animal lifespan studies show promise but don't translate reliably to humans. Sirtuin activation in humans remains largely theoretical.
