Multiple epigenetic clocks (Horvath, Hannum, PhenoAge) show strong correlation with chronological age and predict mortality and disease risk in observational cohorts, but clinical utility remains uncertain. No RCTs demonstrate that tracking biological age or acting on test results improves health outcomes. The tests measure something real, but whether they guide actionable intervention is unproven.
Biological age tests estimate cellular and tissue-level aging by measuring epigenetic markers (DNA methylation patterns), telomere length, or other molecular clocks that correlate with chronological age and disease risk. These biomarkers reflect cumulative damage from oxidative stress, inflammation, and cellular senescence.

Epigenetic clocks were trained on cross-sectional data and may not capture true aging rate in individuals. Different clocks give different estimates and sometimes disagree on whether an intervention worked. No evidence that knowing your biological age motivates or enables better health decisions than standard risk factors (blood pressure, cholesterol, fitness). Telomere length is less predictive than epigenetic markers and highly variable. Most clocks are proprietary, limiting independent validation.
