Routine metabolic and hormone screening is standard clinical practice with strong observational support for identifying disease (thyroid dysfunction, diabetes, dyslipidemia, adrenal insufficiency) and guiding treatment. Abnormal results correlate with morbidity and mortality across large cohorts. However, the evidence base is for *diagnosis and monitoring of known conditions*, not for asymptomatic screening in healthy individuals or for optimizing hormone levels within normal ranges. The clinical utility of comprehensive panels in asymptomatic people remains debated.
A comprehensive hormone and metabolic blood panel measures circulating levels of hormones (thyroid, cortisol, sex hormones, insulin) and metabolic markers (glucose, lipids, electrolytes, liver/kidney function) to identify dysregulation, deficiency, or disease. Most evidence supports screening in symptomatic patients or high-risk groups. Routine screening of asymptomatic individuals for subclinical hormone imbalances lacks RCT evidence that treatment improves outcomes. Overdiagnosis and overtreatment of borderline abnormalities (e.g., subclinical hypothyroidism, mild cortisol elevation) is a recognized risk. Panel results require clinical context; isolated abnormal values without symptoms often do not warrant intervention. Cost-effectiveness of comprehensive screening in healthy populations is not established.
