Most RCTs are small (N < 100) and many use different bacterial strains, making meta-analysis difficult. Placebo response is high in microbiome studies. Long-term safety and durability of microbial changes are poorly characterized. The microbiota is highly individual; a supplement effective in one person may be inert in another. Marketing often conflates mechanistic plausibility with clinical benefit. No strong evidence that probiotics prevent disease in healthy people or reverse aging.
