Egg freezing is an established clinical procedure with decades of observational data showing successful thaw rates (>90%), fertilization rates comparable to fresh eggs, and live birth rates that depend primarily on maternal age at freezing rather than storage duration. Multiple large cohort studies and society guidelines (ASRM, ESHRE) affirm efficacy and safety, though cumulative pregnancy success still reflects the age-related oocyte quality at the time of freezing.
Egg freezing (oocyte cryopreservation) uses controlled-rate cooling and cryoprotectant agents to preserve the metaphase II oocyte at sub-zero temperatures, halting meiosis and metabolic activity. Thawed eggs can be fertilized via in vitro fertilization (IVF), allowing women to defer pregnancy while maintaining reproductive potential at the age the eggs were frozen.

No randomized controlled trials exist (the intervention is not ethically amenable to RCT design). Success rates decline sharply with advancing maternal age at freezing; a 40-year-old's frozen eggs carry the same aneuploidy risk as fresh eggs from that age. Cumulative live-birth rates per egg are lower than per fresh cycle, and multiple freeze-thaw cycles may further reduce viability. Long-term follow-up data on offspring health remain limited, though available studies show no increased birth defects. Cost and emotional burden are substantial and not captured in efficacy metrics.
