Multiple RCTs have demonstrated that 0.1% adapalene is effective for mild-to-moderate acne, with efficacy comparable to tretinoin and benzoyl peroxide. The compound is well-established as a first-line topical retinoid, with good tolerability and a lower irritation profile than earlier-generation retinoids. Response typically emerges over 8–12 weeks.
Adapalene is a third-generation retinoid that binds to retinoic acid receptors (RARs) on skin cells, promoting cell turnover, reducing comedone formation, and decreasing inflammation. It normalizes follicular keratinization and has some antimicrobial activity against Cutibacterium acnes (formerly Propionibacterium acnes).

Efficacy in severe acne is less robust. Adapalene carries standard retinoid caveats: photosensitivity requiring SPF 30+, pregnancy category C (generally avoided in pregnancy), potential initial retinization (dryness, peeling, erythema in weeks 1–4). Efficacy depends on adherence and sun protection. Not superior to benzoyl peroxide monotherapy for most mild acne; combination therapy (adapalene + benzoyl peroxide) is more effective than either alone.
