Pterostilbene shows promising antioxidant and anti-inflammatory effects in cell and animal models, including lifespan extension in some organisms and neuroprotective activity in rodent studies. However, there are no randomized controlled trials in humans, and the translational gap between animal models and human efficacy remains substantial. The evidence is real but confined to the laboratory and animal research.

Mechanism

Pterostilbene is a polyphenolic compound structurally similar to resveratrol that acts as an antioxidant and sirtuin activator. It crosses the blood-brain barrier more readily than resveratrol and may modulate NAD+ (nicotinamide adenine dinucleotide) metabolism and mitochondrial function, potentially supporting cellular stress resistance and longevity pathways.

Caveats

No human RCTs exist for pterostilbene supplementation. Most human data are observational or mechanistic in vitro studies. Bioavailability in humans is poorly characterized compared to resveratrol. Animal lifespan studies are limited and often use non-standard strains. The relevance of sirtuin activation in mice to human aging is unproven. Marketing claims about longevity or cognitive enhancement lack human trial support.
