Metformin has strong RCT evidence for glycemic control in type 2 diabetes and observational data suggesting cardiovascular and mortality benefits in diabetic populations. The TAME trial (Targeting Aging with Metformin) is ongoing in non-diabetic older adults to test longevity claims, but results are not yet published. Current evidence supports use in diabetes; longevity benefits in healthy people remain mechanistic.

Mechanism

Metformin activates AMP-activated protein kinase (AMPK), a cellular energy sensor that regulates glucose metabolism and mitochondrial function. It reduces hepatic glucose production and improves insulin sensitivity, lowering blood glucose levels. In aging research, AMPK activation is hypothesized to extend healthspan by improving metabolic efficiency and reducing inflammation.
